SURMOUNT-3: Tirzepatide After Lifestyle Intervention — Maximizing Results
SURMOUNT-3 studied tirzepatide in patients who had already lost weight through diet and exercise. The results show even greater total weight loss.

Study Design
- Phase 1 (Lead-in): All participants underwent 12 weeks of intensive lifestyle intervention (low-calorie diet + exercise)
- Phase 2 (Randomization): Those who lost ≥5% body weight were randomized to tirzepatide (maximum tolerated dose) or placebo for 72 additional weeks
- Participants: 579 adults with BMI ≥30 or ≥27 with comorbidities
Results
During Lead-in (12 weeks)
- Average weight loss: -6.9% body weight from lifestyle intervention alone
After Randomization (72 weeks)
- Tirzepatide group: Additional -18.4% body weight (total: -26.6% from original baseline)
- Placebo group: Regained weight, ending at -2.1% from randomization
Key Findings
- Total weight loss from original baseline exceeded any previous anti-obesity medication trial
- 94% of tirzepatide patients lost ≥5% additional body weight
- 29% achieved ≥30% total weight loss from baseline
- BMI reduction: from average 38.1 to average 28.1 (moving many patients out of the "obese" category)

Sequential Approach Works
This trial proved that a "lifestyle first, then medication" approach produces the best total results. The combination of behavioral changes plus medication exceeded results from either alone.
